Project Objectives [from section 2 of PCN] The objective of the project, which would remain unchanged, is to contribute to behavior change among the Chadian population in order to reduce the risk of both HIV infection and too closely spaced and/or unwanted pregnancies. The project finances five components: (1) support to a multisectoral response to HIV/AIDS through strengthening the capacities of key ministries; (2) support to the Social Fund (FOSAP) and community interventions focused on behavior change and capacity building of community-based organizations; (3) social marketing of key health- related products; (4) support to the implementation of the National Population Policy; and (5) support to project implementation and monitoring and evaluation. C. Project Description [from section 3 of PCN] The proposed additional financing would permit consolidation of recent gains on the ground and allow for the scaling-up of the local integrated response to HIV/AIDS including community interventions, such as peer education and IEC programs targeting most at risk groups. The additional financing would also serve as a bridge until the preparation of the new MAP project in 2009 as well as allow the strengthening of the quality of current interventions, the production of Page 2 concrete results on the ground (which would also feed into a more thorough final evaluation exercise) and the identification of lessons learned from experience for the preparation of a new MAP project. In doing so, the additional financing will also help tackle the following emerging issues: (i) persistent stigma and discrimination against people living with HIV across the country; (ii) gender inequality in accessing health services; (iii) weak capacity; and (iv) insufficient funding to contribute to the achievement of the health-related Millennium Development Goals. The new approach is expected to be introduced into two new HIV/AIDS high-risk areas, one of which is a conflict-affected area. To this end, partnerships will be strengthened with other donors, such as UNICEF, and civil society, such as Medecins Sans Frontiers, working in these areas to ensure a coordinated and harmonized approach to health services in these areas. Moreover, the capacity building element of the new approach would be expected to contribute to strengthened capacity of local levels of Government to manage health services. No new construction and/or rehabilitation of facilities will take place under the proposed additional financing operation. The implementation of the National Medical Waste Management Plan will be supported under component 5. D. Project location (if known) The project will be continued in Moundou, Ndjamena and Mao and scaled up to two additional high risk areas. E. Borrower’s Institutional Capacity for Safeguard Policies [from PCN] The Borrower's institutional capacity for implementing safeguard is somewhat weak as they do not have much experience. Capacity building on safeguards, particularly medical waste management, will be built into the additional financing under component 5, to address this weakness. F. SAFEGUARD PREPARATION PLAN A. Target date for the Quality Enhancement Review (QER), at which time the PAD-stage ISDS would be prepared: N/A B. For simple projects that will not require a QER, the target date for preparing the PAD-stage ISDS: 07/23/2007 C. Time frame for launching and completing the safeguard-related studies that may be needed. The specific studies and their timing 1 should be specified in the PAD-stage ISDS. The Medical Waste Management Plan will be prepared and disclosed prior to appraisal scheduled for July 24, 2007. An international consultant has been recruited and has begun work on this task. The team has identified an experienced safeguard specialist from ASPEN that will provide the necessary guidance and support the implementation of MWMP. IV.
