For the first time, the SOPE report will include recipient- executed projects (over US$ 5 million). The World Bank has issued a SOPE Report every year as an internal communication to the Board of Executive Directors. In accordance with the Bank's revised disclosure policy, since FY02, the SOPE Report is available to the public. The report is intended to bridge the gap in information available to the public between the Project Appraisal Document or Program Document, disclosed after the Bank approves a project, and the Implementation Completion Report, disclosed after the project closes. Progress toward achieving development objective(s): After completing the first 12 months of a proposed two-year extension, the project accounts the approval by the Uttar Pradesh (UP) Assembly of the Bill establishing an autonomous Water Regulatory Commission which is an institutional modernization step. The milestone set forth for the modernization of the infrastructure in the pilot area (300, 000 hectares) is also on track. All contracts for canal and drains are under implementation, improving the irrigation services to 500 well-established Water User Associations. The modernization of the irrigation sub-sector process includes the computerization of the UP Irrigation Department, implementation of web-based monitoring and evaluation and project management information systems. Considering the strong political support, project development objectives are likely to be achieved in case the second extension through October 2009 is granted. Progress toward achieving development objective(s): Project implementation performance is continuing to progress steadily with US$77 million disbursed in fiscal year 2008 compared to US$44 million in fiscal year 2007. Significant progress has been made in delivering key outputs (road upgrading, road maintenance, institutional strengthening). Steps have been taken to initiate long-term performance based maintenance contracts, road Public Private Partnerships and to deliver tools that will improve planning and budgeting of road network maintenance and development. However, because of a significant financing gap due mostly to variation in exchange rates and price escalation, additional resources are required to complete the project, deliver outputs and achieve project development objectives (PDOs) as agreed at appraisal. Two performance indicators measuring PDOs have reached their end-of-project (EOP) targets (road accident facilities and road maintenance budget). Two other indicators seem to confirm that the project is on the right track to achieve EOP targets (improvement of road network condition and reduction in travel time). Actions have been taken to improve the indicator measuring implementation of the Institutional Strengthening Action Plan. To improve access of below- poverty-line populations to health care, outreach camps are being routinely conducted in nine tribal/dessert districts. Counselors are in place in all 50 bedded and above project hospitals to facilitate uptake of services of the disadvantaged groups. The project has built a significant capacity for facilitating and conducting administrative, clinical and behavioral training in the state. This capacity is being shared with the Department of Health and Family Welfare, and would support longer-term sustainability of such inputs. Impressive performance is recorded in rationalizing and institutionalizing Hospital Management Information Systems and generating complete, regular data from 100% of project facilities on indicators developed to measure their performance. Hospital Systems Improvement Teams have been constituted in each project facility and the cyclical process of problem solving has resulted in improvement in hospital system, quality of care and management. The project continues its efforts to facilitate policy dialogue between various stakeholders and implementation of National Bio-medical Waste Disposal rules at Government hospital level. In 2000 Uttaranchal (now Uttarakhand) was established as a separate State from Uttar Pradesh. Progress toward achieving development objective(s): The agreed indicators (increased health sector spending, increased number of out-patient and in-patient in project facilities and increase in institutional deliveries) for achieving the project's development objective have been reached in Uttarakhand. As for Uttar Pradesh, although some indicators had been reached (out-patient and in-patient numbers) the achievement could not be contributed to the project as the increase was not significant when adjusting for the increase resulting from a substantial reduction in user fees. The project was therefore extended and re-oriented to have a population rather than facility focus. For the first two years of project extension there was good progress and preparatory activities for the follow-on project were under way. A third extension year was planned to ensure readiness for the follow-on project. This extension was however delayed for almost three months for reasons external to the project. This delay and associated uncertainty about the realism of a follow-on project led to substantial delay in the proposed activities which may therefore not all be completed by project closing. This situation is likely to negatively impact the achievement of stated objectives for the re-oriented project in the extension phase. The efficiency of sub-urban train operation has increased with a rise in peak-time capacity by 7-10 percent and a reduction in commuting time by 4-9 minutes for about six million citizens as a result of relocation of thousands of squatter families living along the rail tracks and track doubling. Improvement in transport service in terms of quality and comfort is noticed as a result of the addition of new trains and replacement of 644 old buses by new ones. Most of the activities will require an extension for project completion. In this respect, the Borrower has presented a request to extend the closing date along with a restructuring package that aims at reducing the scope of the project to take into account the appreciation of the Indian Rupee compared to the United States dollar and at focusing the scope of project on activities that can be realized within a reasonable time period. Progress toward achieving development objective(s): Most of the project implementation has been completed and the focus now is on achieving proper closure of the project. The Project has successfully completed more than 3600 small and 6 large water supply schemes benefiting over 173, 000 families. Achievement in improving household sanitation coverage and use are also impressive. Of the beneficiary population, 53% are below poverty line families, and 16% are Schedule Caste families. In addition, 1000 tribal families have benefited from the project - all surpassing the Project Appraisal Document targets. In addition, alternate institutional and financing models have been developed and piloted which would contribute to Government of Kerala's aim of scaling up of the new approach. In achieving the above results, The implementing agencies and Non Government Organizations are successfully playing the role of facilitators in Rural Water Supply and Sanitation service improvement, by building local capacity in service management. Evaluation of sustainability of the schemes so far carried out in about 1500 water supply schemes by external consultants indicates that over 90% of the schemes are likely to be sustainable. Thus, the project has demonstrated successfully not only the decentralized delivery, but has proved to be inclusive and equitable. These achievements provide evidence of likelihood of achieving development objectives of the project. The CFC producers have stringently and consistently met their production targets, despite a scenario of reduced international supply of CFC, and a continued demand in the domestic market for servicing of existing equipment and for production of metered dose inhalers. The CFC production phase-out component has been successful in reducing total production to 2, 346 metric ton in CY2007, well within the annual production cap of 3, 389 metric ton. The Government of India has taken a decision to accelerate CFC production cessation to August 1, 2008, 17 months ahead of the target as established by the Montreal Protocol. Once the modalities of this accelerated phase-out are finalized with the Executive Committee of the Montreal Protocol, the activities under this project will be modified to address the revised monitoring, enforcement and public awareness requirements, in collaboration with the Government of India. The project was restructured, or received Additional Financing during FY08 The size of the Additional Financing is SDR12.1 million (US$18 million equivalent) and its scope is to assist in bridging the financing gap which resulted from a cost overrun in the road improvement component of the project. The expected outcome from the activities to be supported with additional financing would be sustainable improvement in management and carrying capacity of the core state road network. Progress toward achieving development objective(s): Substantial travel time and cost savings have been achieved for the road maintenance sub-component which has been substantially completed since last year. On the other hand, implementation of the road improvement component has been seriously delayed and only about 80 kilometers (km) (out of 182 km) stretch has been completed to the final stages. With the present rate of progress, completion of the remaining road improvement works could only be achieved by mid- to end-2009, thereby delaying realization of the expected travel time and cost benefits associated with road improvement works by two to two-and-a-half years. The road safety component is expected to be fully implemented by end-2008 resulting in improvement of the overall safety environment and reduction of road fatalities. The Public Works Department-centered institutional strengthening program involving modernization and reform- based targets in road sector policy, legislation, financing, project and maintenance management, office computerization, re-organization and skills development has been about 80 percent completed and expected to substantially achieve the end-of-the-project targets by early 2009. The project was restructured, or received Additional Financing during FY08 The amount of Additional Financing is US$64 million (50 percent IDA and 50 percent IBRD). The objective and scope of the Additional Financing are similar to the original credit with three components: (a) establishing an enabling environment for the sustainable, decentralized management of tank systems; (b) strengthening community-based institutions to assume responsibility for tank system development and management; and (c) undertaking tank system improvements. The Additional Financing expands the project scope to eight additional districts and aims to work on about 1, 200 tanks. Progress toward achieving development objective(s): Communities around about 1, 000 tanks have started to benefit from project activities. Cropping intensities in a sample of rehabilitated tanks increased by 21 percent to slightly over 100 percent. Project yields increased between 8 percent for groundnut and 13 percent for sunflower. Household incomes of direct beneficiaries in the sample tanks saw their income increase by 17 percent to Rs. 34, 755 on average. Various income generation activities were implemented in 1, 138 tank areas, covering 48, 234 families, and providing substantial benefit to poor households. All these achievements show that rural livelihoods are improving and poverty is reducing in tank communities. Progress toward achieving development objective(s): The location of the central surveillance unit in National Institute for Communicable Diseases ensured enhanced technical support for the project and 23 states now have Surveillance Units established at state and district levels. A large number of health staff have been trained in standard operating procedures. The project is currently focusing on 14 states to demonstrate effective surveillance and enhance coordination with national disease control programs. Information on priority diseases is being regularly reported by rural primary health facilities from over 90% of 252 districts belonging to the 14 focus states. Weekly outbreak reporting has started and 20 states have started reporting outbreaks and documenting responses, even though improvements are required in the areas of laboratory confirmation, and quality and timeliness of response. The non communicable disease risk factor surveys are in progress in 8 states and preliminary results are expected by September 2008. Baseline assessment of laboratories and external evaluation of training have been completed. With technical support from WHO, a new two week training program on field epidemiology is being implemented. Despite these improvements, obtaining surveillance information regularly from the larger public hospitals and private sector still remains a challenge. The initiative started to rationalize the weekly reporting forms to reduce the burden of nonspecific conditions on the surveillance system is still to be implemented. More importantly, the ability to analyze and act on the information being generated is critically lacking especially at the district level. Similarly, due to limited availability of microbiologists, the original plan for strengthening public health laboratories could not be implemented. The Government of India has initiated efforts to address these operational constraints by creating positions of epidemiologists under the National Rural Health Mission and scaling down the laboratory strengthening component. There is a gradual increase in the uptake of services offered by nongovernmental organizations by way of public private partnerships to provide mortuary van, referral transportation and counselor services in project facilities. Activities planned under the Non-Communicable Disease pilots are being implemented. Cancer Cervix pilot is being fine-tuned to ensure follow-up and complete compliance. The response of audience from school and work place based cardio-vascular disease pilots is encouraging. Software development and pilot implementation of both the Health Management Information Systems and Hospital Management Systems is underway. The Quality Council of India is facilitating the accreditation process of 12 Government hospitals. In addition to initiating training in health care waste management (HCWM), Common Treatment Facilities have been contracted across the state for final disposal of bio-medical waste. This will ensure state-wide operationalization of HCWM from September 2008. In collaboration with the Government of Germany, a manual and a training program have been developed for advising users on environment and health and safety guidelines for CTC alternatives. Far-reaching legal reforms designed to transform the governance and operational efficiency of credit cooperative banks (CCBs) were implemented across several states. The process of recapitalization proceeded as envisaged and a large number of primary agricultural credit societies (PACS) were recapitalized as per the agreed eligibility criteria. Training was delivered to PACS officials, new Management Information and Common Accounting Systems were developed and progress was made with developing PACS capacity to use these systems. As a result of these measures, progress was recorded during fiscal year 2008 with the achievement of the project's development objectives. For instance, both ground-level financing (credit) extended to farmers through CCBs and the number of small and marginal farmers borrowing from CCBs increased in line with project targets. Recovery rates of CCBs improved during fiscal year 2008 although the debt waiver scheme announced by the Government in late February is likely to make recovery more difficult over the medium term. Overall profitability of CCBs also increased. Elections were held for the first time in many years to reconstitute the Boards of CCBs.
