[ x] Environmental Category: B Safeguard Classification: S2 Project Objective: Support the national program that aims to prevent and to control the spread of HIV/AIDS and to mitigate the socio-economic impact of the disease on the population. The project will use a two pronged strategy: targeting interventions at high risk groups and implementing non-targeted activities for the general population. Successful achievement of the development objective will: (a) have high risk groups use safe sexual practices; (b) enhance knowledge in the general population of the negative societal and family impact of the disease; (c) prolong the lives of infected persons and provide care and support to their families; and (d) reduce the degree of stigma and discrimination associated with the disease. A.3. Project Description Com p onent 1: Scalin g u p HIV/AIDS res p onse b y Civil Societ y Or g anizations. The p ro j ect will Page 3 empower communities to respond effectively to the HIV/AIDS epidemic. Civil society groups such as Non Governmental Organizations (NGOs), community based organizations (CBOs) faith- based organizations (FBOs) and the private sector have access to different groups including difficult-to-reach groups that are particularly high risk (commercial sex workers, men having sex with men, prisoners, truck drivers) and vulnerable groups (orphans and youth). Activities to be implemented by these organizations include: behavior change communication; support to orphans, provision of care for PLWHA; and distribution of condoms. The project will also finance capacity building for the CSOs. Component 2. Scaling up the response by line ministries. The project will support Line Ministries to expand initiatives in accordance with priorities in the National Strategic Plan for AIDS 2004-2009. The project will support crosscutting activities focusing on prevention of HIV/AIDS and STIs through: training, information, education communication/ behavior communication; condom distribution; treatment and care for the infected and affected families; work place policy formulation including reduction of stigma and discrimination; and, HIV/AIDS impact assessments. There are also HIV/AIDS related interventions that are specific to a each ministry’s external clients, for example, students, teachers and parents for the Ministry of Education, the hotel and restaurant industry for the Ministry of Tourism, and PLWHA’s, orphans and school drop outs for the Ministry of Social Development. Interventions will be tailored by each of the Ministries to their respective clients. Component 3. Expanding Health Sector Prevention, Treatment and Care Services for HIV/AIDS. The project will support strengthening, upgrading and expansion of prevention, treatment and care services through the health care system. This will include upgrading of laboratories, voluntary counseling and testing rooms, training health care workers and counselors and provision of equipment, drugs, supplies, testing kits, condoms, and technical assistance in key areas: Information, Education, Communication/Behavior Change Communication; Voluntary Counseling and Testing (VCT); condoms distribution; Treatment (sexually transmitted diseases and opportunistic infections treatment including antiretroviral treatment; Laboratory services; and, Biomedical waste management. Component 4. Institutional Capacity Strengthening, Monitoring, Evaluation and Research The project will support institutional capacity building for scaling up the response through financing of technical advisory services, training, staffing, equipment, goods and general operating costs for strengthening institutional capacity for coordinating and managing the Government’s National HIV/AIDS Program and; Strengthening program monitoring and evaluation. The Ministry of Health and Environment has oversight of biomedical waste management. In 2002, a National Biomedical waste Management Plan was prepared for St. Vincent and the Grenadines. This plan was a component of a larger World-Bank-funded program to address the problem of solid and ship-generated waste. The program covers six OECS countries and is coordinated by the Natural Resources Management Unit of the OECS. The plan was adopted by the St. Vincent and the Grenadines Government in April 2002. The plan is under implementation. The Ministry of Health and Environment, the St.Vincent and the Grenadines Water and Sewerage Authority and the health care facilities have the capacity to implement the waste management plan. D.5. Identify the key stakeholders and describe the mechanisms for consultation and disclosure on safeguard policies, with an emphasis on potentially affected people. The biomedical waste management report was prepared through consultations with technical staff in the St. Vincent Water and Sewerage Authority, health care workers at different health care facilities and officials of the Ministry of Health.
