It describes the procedure to be followed and processes to ensue before they can withdraw and after they have withdrawn from the impact evaluation. 2 Withdrawal Definition Withdrawal from the impact evaluation refers to a process whereby a participant (if 18 or older) or a participant’s parent / caregiver (if a participant is younger than 18) informs the impact evaluation team that they no longer wish to participate. This could be a personal decision or the parents revoke assent for the participant to be part of the impact evaluation , (for participants under 18 years). 3 Procedure (a) Participant (or her caregiver / parent, if younger than 18) advises a Sitakhela Likusasa impact evaluation team member (it could be field manager, NERCHA contact person, research assistant or other person from NERCHA, IHM or the WB in the study team) that they no longer want to continue participating in the impact evaluation. (b) Sitakhela Likusasa impact evaluation team member to inform the participant (or her caregiver / parent, if younger than 18) that they will be contacted by the Project manager or Assistant field manager to discuss the withdrawal procedure. (c) The impact evaluation team member to log the study withdrawal request in an email (participant name (or her caregiver / parent, if younger than 18), circumstances of withdrawal request, cell phone number where to contact participant and inform the Project manager (Vimbai Tsododo), Assistant field manager (Biomedical) (Tendai Chipepera), Assistant field manager (Education) (Leroy Shongwe) and NERCHA HIV prevention manager (Mzwethu Nkambule). (d) The Project manager (PM) or Assistant field manager (AFM) contacts the participant (or her caregiver / parent, if younger than 18), and perform counselling about reasons for and implications of withdrawal from the impact evaluation. If the participant is younger than 18, the parent and participant needs to be counselled separately. (e) Based on the outcome of the conversation, PM or AFM to confirm study withdrawal or notes that participant decided not to go through with it. (f) If participant decides to go ahead with withdrawal, then an appointment will be made for a date and time for the PM or AFM to visit the participant to get the withdrawal form signed & to return the participant ID to the PM or AFM. (g) Field Manager to get withdrawal form signed by the participant (or her caregiver / parent, if younger than 18) and if possible, get the participant ID. Page 2 (h) AFM or PM to hand signed form to Sitakhela Likusasa Data Manager, who will log the withdrawal in the master impact evaluation database, and to capture on the form the date that the withdrawal was captured in the same master database. (i) Signed withdrawal form to be scanned, uploaded to the one-drive and filed for archiving. (j) All future versions of the database and Survey Solutions forms to exclude the withdrawn participant. (k) Though a participant withdraws from the impact evaluation, they should remain in the study database and not be deleted.
