Re:    Reimbursable Advisory Services Agreement for National Health Fund
First Amendment Letter
Dear Mr. Milkowski:
We refer to the Reimbursable Advisory Services Agreement between the National Health
Fund of Poland (the "Client") and the International Bank for Reconstruction and Development
(the "Bank"), dated November 30, 2015 ("Agreement"), and to the recent discussions between the
representatives of the Client and the Bank regarding extension of the expiration date of the
Agreement.
In accordance with the above referenced discussions, the Bank hereby amends
(modifications shown in italics):
1. Paragraph 6 of the Agreement to read as follows:
"6.    Expiration. This Agreement shall expire on June 30, 2017, unless it shall earlier
be renewed with the mutual agreement of the Client and the Bank."
2. The table in Section of the Schedule of the Agreement to read as follows:
B. Timetable. Except as the Client and the Bank may otherwise agree, the Bank will perform the
Reimbursable Advisory Services in accordance with the following tentative timetable:

1. Review report of the key domains of the IDS.
Component B:                                                June 2016
2. Inception report presenting the ICO pilot design and
implementation models.
3. Final report with the models for the pilot design.       May 31, 2017

-Proposed technical programs of the study tours
- Technical contribution to the information materials produced by
the NFZ.
- Presentations during the PSTs meetings and national workshops
and conference organized by the NFZ (max 3 per year)
Except as amended herein, all the other provisions of the Agreement shall remain in full
force and effect.
Please confirm the agreement of the Client to the amendment set forth above by
countersigning and dating both copies of this Amendment Letter and returning one of the
countersigned copies to us. Upon receipt by the Bank of the copy of this Amendment Letter
countersigned by you, this Amendment Letter will become effective as of the date of the
countersignature.
